Mostly the “drives me mad” sense.
In speech you will hear rendre above all in the emotional sense: « ça me rend dingue », « ça me rend ouf » (ouf is verlan for fou). Both mean it drives me crazy, and both are completely ordinary among friends.
« Rends-moi ça ! » — give that back — is the everyday demand, and « tu me rends mon truc ? » is its softer version. The ne goes as usual.
